Care Coordination Registered Nurse (RN) to provide exceptional
patient care and support care coordination efforts. This contract
position requires strong nursing skills and the ability to work
both day shifts and some weekends. Responsibilities: Coordinate
patient care plans to ensure seamless transitions across healthcare
services Collaborate with interdisciplinary teams to optimize
patient outcomes Maintain accurate and timely documentation in
compliance with regulatory standards Educate patients and families
on treatment plans and follow-up care Monitor patient progress and

California Registered Nurse (RN) license Current Basic Life Support
