Job Summary The Licensed Practical Nurse (LPN) is responsible for delivering professional nursing care to residents of all ages in a long-term care setting, under the guidance of a registered nurse, advanced practice provider, or physician. The LPN will apply nursing principles relevant to the clinical environment and demonstrate proficiency within the full scope of nursing knowledge. This role involves utilizing appropriate standards tailored to the physical and psychosocial needs of residents as outlined in their care plans. The LPN will: Ensure the health, comfort, and safety of residents. Participate in resident assessments and administer medications or treatments as prescribed. Provide technical support to healthcare professionals as required. Document interactions with residents and outcomes related to care, medication, behavior, activities, and dietary needs. Build and maintain effective relationships with residents, healthcare providers, and the community. Collaborate effectively with interdisciplinary care teams. Communicate clearly while maintaining confidentiality of sensitive information. Advise residents and their families on prevention and treatment plans under the direction of a registered nurse or physician. Utilize knowledge of resident education principles to create self-care programs. Perform additional related duties as assigned. Qualifications Graduation from an accredited practical nursing program as a Licensed Practical Nurse; an Associate degree in practical nursing is preferred. If IV therapy/management skills are required, candidates must have graduated from a Board-approved AASPN program or completed an IV Therapy course approved by the relevant nursing board. Completion of a pre-licensure nursing program that included supervised clinical experience across the lifespan, or a minimum of 400 hours of experience as an LPN in another facility. Specific roles may have additional educational or training requirements. Must hold an unencumbered LPN license with the State Nursing Board and/or possess multi-state licensure privileges. Must obtain and maintain required competencies and certifications specific to the department. Complete annual continuing education units (CEUs) and competency training as mandated by the department and facility.
