This position offers a unique opportunity to make a profound impact on patients' lives while working in a supportive and technologically advanced environment. Provide daily living assistance and basic healthcare to patients in their homes. Monitor patient health status and report any significant changes to the assigned supervisor or healthcare professional. Work collaboratively with a multidisciplinary healthcare team to implement patient-specific care plans. Educate patients and their families on health maintenance and preventive measures. Ensure compliance with all health care regulations and maintain accurate patient records. Assist with the administration of medications as prescribed by the healthcare provider. Strong interpersonal and communication skills Proficiency in using healthcare software and systems Ability to perform basic healthcare tasks (e.g., taking vital signs, providing personal hygiene) Capacity to handle physical demands of the job including lifting and moving patients Excellent organizational and time-management skills Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Certified Nursing Assistant (CNA) or Home Health Aide (HHA) certification preferred. CPR and First Aid certification required.
